Volunteer Waiver

Release and Waiver:

Volunteers hereby release and forever discharge and hold harmless Family Support Center (FSC) and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from volunteer's activities with FSC. Volunteers understand that this Release discharges FSC from any liability for claim that the volunteer may have against FSC with respect to any bodily injury, personal injury, illness, death, or property damage that may result from the volunteer's activities with FSC, whether caused by the negligence of FSC or its officers, directors, employees, or agents or otherwise. Volunteers also understand that FSC does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health, or disability insurance in the event of injury or illness.

Assumption of the Risk and Medical Treatment:

The volunteer understands that the activities include work that may be hazardous to the volunteer, including, but not limited to, construction, loading and unloading supplies, and transportation to and from the work sites. Volunteer hereby expressly and specifically assumes the risk of injury or harm in the activities and releases FSC from all liability for injury, illness, death, or property damage resulting from the activities, this includes but is not limited to exposure to COVID-19. Volunteers hereby release and forever discharge FSC from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with the volunteer's activities with FSC.

Photo Release:

I consent to being photographed, interviewed, and/or videotaped by employees and/or representatives of FSC. I authorize the FSC, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that any information or images obtained from those activities may be reproduced by FSC, its assigns and transferees for any lawful purpose, including but not limited to use in advertising, publicity, or educational activities, websites, publications, videos, print, and television news. I hereby waive any claims I may have, and release the Family Support Center, its employees, representatives, assignees and transferee's from liability of claims arising out of such activities.

Temporary guest passes are valid for one day only, on a case by case basis. Temporary volunteers have not passed background checks and are therefore NOT permitted to participate in any activity with children unsupervised nor perform any service that may result in access to sensitive information.
